Doctor and dentist registration
In registering with your local doctor and dentist you will need the following information.

1. NHS number.
2. Details of your old doctor and dentist surgery (name, address and telephone number).
3. Details of your new address.
4. Any other information which may be important before they receive your medical records, such as allergies or illnesses you may still have.

If you need to visit the NHS but have not yet joined your local surgery or dentist there are a variety of options available to you.

1. Visit your local NHS walk in centre.
2. Go to your local surgery and sign on as a temporary patient until you have a chance to register officially.

4. Visit your local pharmacist who maybe able to help you, you can find this by searching for your local pharmacy on-line at the NHS site, local services or by searching within the chemists and drug stores section of the guide.
